Nature of Position

The person in this position will have a natural disposition and friendliness towards seniors.  They will work with and support seniors, with the goal of decreasing frailty, based on the CARES Model.  They will work one-on-one with seniors to coproduce a personalized support plan, and then encourage and motivate them to participate in activities/programs in the community, as outlined in their personalized plan, to improve their overall mental and physical health, which may include activity/exercise groups, presentations and workshops, counselling, social group activities, and health and wellness clinics.

They will work with United Way researchers on an ongoing basis to measure the impact of the program on seniors, as well as participate in United Way meetings, conferences and Communities of Practice meetings.  They will work closely with the Division of Family Practice and local physicians to receive referrals and maintain follow-up communication with them regarding progress on the seniors they’ve referred.   They will also recruit, train and oversee volunteers, who will support seniors to participate in activities, as outlined in their Service Plan.

Qualifications (education and experience)

A Diploma in a related human/social service field.

Must have a minimum of two years’ experience working directly with frail and vulnerable seniors.

A minimum of two years experience with volunteer coordination.

Current First Aid/CPR, and Non Violent Crisis Intervention

Clean Criminal Record Check

Valid Drivers License, a minimum of 5 million vehicle liability insurance with business class, and a clean drivers’ record.

Required Skills (knowledge and skills)

The ideal candidate is a strong team player with excellent interpersonal skills

Must have excellent computer/technological skills with “iunite” database

Ability to make effective referrals, and have an in-depth knowledge of community, government and financial resources for seniors and strong collaborative skills

Must possess excellent written/verbal communication and presentation skills. 

Excellent computer/social media skills (including social media, CAMS, Office 2013)

Ability to work independently

Excellent verbal, active listening, and communication skills

Knowledgeable about Community Services, accreditation practices, performance quality improvement, client and program records and documentation.

Excellent organizational skills, file management, scheduling, and time management.

Nonjudgmental, compassionate and culturally accepting.

Behaviour change skills

Motivational interviewing and coaching skills

Shift/Schedule/Hours per week:  Casual position to May 30, 2021  24 hours per week (flexible schedule)

Wage/Salary: starting at $23.23/hour
